What is Swasthya Sathi Scheme ?

From 17 February 2016, under the initiative of the Government of West Bengal and the guidance of Hon’ble Chief Minister Mamata Banerjee, the Swasthya Sathi Scheme was launched to improve public health and ensure proper medical treatment. By registering under this scheme, all members of a family can receive financial insurance coverage of up to ₹5 lakh per year.


Benefits of the Swasthya Sathi Scheme

1) Yearly coverage of ₹5 lakh

Under this scheme, you and your family are eligible to receive up to ₹5 lakh annually for health-related expenses funded by the State Government.

2) Coverage for the entire family

The beneficiary woman, her husband, children, parents, in-laws, and dependent family members are covered under the scheme, and can avail treatment using the card.

3) Premium

The entire premium for the Swasthya Sathi Scheme is paid by the Government of West Bengal, so beneficiaries do not need to worry about premium payments.

4) Completely cashless hospitalization

From doctor’s consultation to diagnostic tests, medicines, surgeries — all costs are borne by the scheme.
Additionally, the scheme covers the cost of medicines for five days after discharge.
However, food expenses during hospitalization are not covered.

5) Transportation cost after discharge

A transport allowance of ₹200 is provided after discharge.
If admitted to a government hospital, the allowance may range between ₹400 to ₹700.

6) Empanelled hospitals

There are currently more than 1500 empanelled hospitals under the scheme, with new hospitals joining regularly.
Before admission, beneficiaries should check whether the hospital is listed.

7) Pre-authorization for treatment

Pre-authorization ensures quick approval for medical services so that treatment is not delayed.

8) Smart Card

A smart card is issued containing encrypted digital information of all family members.
This allows a patient to receive cashless treatment easily.

9) Online health record

All health-related updates during hospitalization are uploaded online for easy future reference by doctors.

10) Health Packages

There are currently 2092 health packages under the scheme.
Any changes in package structure or pricing are determined by the Swasthya Sathi Committee.

11) Mobile App

You can download the Swasthya Sathi mobile app from Google Play Store to check empanelled hospitals and submit complaints.

12) SMS Alerts

SMS notifications are sent to the registered mobile number when a beneficiary is admitted and discharged from the hospital.

13) Toll-Free Number

Beneficiaries can call the toll-free number (printed on the back of the smart card) 24×7 for assistance.


Eligibility Criteria for Swasthya Sathi Scheme

Those who wish to apply must meet the following criteria:

  • The applicant must be a permanent resident of West Bengal.
  • The senior woman of the family is considered the primary applicant, along with her husband, parents, in-laws, children, and dependent members.
  • Disabled members of the family are also covered.

Who are NOT eligible?

  • Individuals already enrolled under any government health insurance scheme.
  • Persons receiving medical allowances as part of government employment benefits.

Documents Required to Apply

  • Valid ID proof (Aadhaar / Voter ID)
  • Residential proof of West Bengal
  • Family member list (names of members to be included in the card)

How to Apply for Swasthya Sathi Scheme

Offline Application Method

  • Visit the nearest Duare Sarkar camp.
  • Collect the Swasthya Sathi application form.
  • Fill it correctly and attach the required documents.
  • Submit the form at the same camp.

Online Application Method

  1. Visit the official Swasthya Sathi portal.
  2. Click on Apply Online.
  3. Select Online Application for Swasthya Sathi Card.
  4. Enter your mobile number → Click Get OTP → Verify OTP.
  5. Fill out Form B with accurate details.
  6. Enter district/municipality/block information.
  7. Provide employment details (government/private), full address, and PIN code.
  8. Enter the senior woman’s name and add other family members.
  9. Mention if any member is a government employee or already has government insurance.
  10. Provide details like family members’ names, ages, relationships, Aadhaar numbers, etc.
  11. Submit the form.

  1. You will see your current application status.
  2. Visit the official portal.
  3. Go to Apply Online → Select Check Your Online Application Status for Swasthya Sathi Card.
  4. Choose your district and enter your Aadhaar number.
